Incident & Concern Reporting
The IACUC relies on timely reporting to help ensure animal welfare, regulatory compliance, and the integrity of the research program. This page provides information on two types of reporting: Incident Reporting and Concern Reporting.
While these processes serve different purposes, both are important tools for identifying and addressing issues. Use the guidance below to learn how to distinguish between incidents and concerns, when each type of report is appropriate, and how to submit a report.
Types of Reporting
Incident Reporting
Report an incident when an event, deviation, or unexpected occurrence involving animal care, use, or research activities has already taken place and requires review by the IACUC.
Concern Reporting
Report a concern when you observe, suspect, or become aware of a potential issue related to animal welfare, compliance, or research practices that may warrant further evaluation by the IACUC.
